Lithium itself is nephrotoxic, and predisposes patients to chronic toxicity if renal function and dosing is not monitored closely. Patients taking NSAIDs, ACE inhibitors, diuretics or topiramate concomitantly with lithium are at increased risk of chronic lithium toxicity.
